The FREE-STANDING EMERGENCY UNIT (FED)

 

The Damour Region and Western Chouf/Aley are deprived with regard to emergency medical services. This region strongly needs an acute care facility, an "Urgent Care Center" or an "FED", one where we can provide the following ambulatory outpatient services:

 1. Triage and Medical Screening of all walk-in presenting complaints

 2. Outpatient evaluation and management of all acute illness and injuries care

 3. Resuscitation and stabilization of all presenting acute conditions to the point when they can then be transferred by ambulance to a higher level of care

 4. Referral and Transfer of all patients who need inpatient admission to hospitals in the region

 5. Supporting Outpatient Diagnostic Facilities including Laboratory, Radiology, Ultrasonography, Echocardiography, Mammography, and Procedures such as Bronchoscopy, Gastroscopy, Colonoscopy, Cystoscopy and Minor Surgical Procedures

 

The Damour Medical Center FED would be an outpatient urgent care facility where credentialed physicians on-duty 24/7 can practice emergency care delivery to presenting acute illnesses and injuries, providing triage, patient evaluation, outpatient management, including:

1.   Screening of critical or potential critical patients, prompt resuscitation, stabilization and qualified transfer to a higher level of care

2.   Referral of patients who need inpatient care through a qualified coordination and execution of transfer after initial care and diagnostic workup has been initiated

3.   Discharge home of patients who have been treated and can go home

 

There will of course be:

1.  No general anesthesia administration

2.  No major surgery that requires an OR block

3.  No intensive care unit

4.  No interventional cardiology or gastroenterology or pulmonary procedures

5.  No interventional radiology

7.  No inpatient admission for more than 24 hours of observation

 

Procedures to be conducted & equipment to be used in the FED include:

1.  Suturing of Lacerations

2.  Initial Wound & Burn Care

3.  Drainage of Superficial Abscesses & Minor Surgeries

4.   Orthopedic Splinting & Casting + Relocation of Dislocated Joints

5.   Emergency Endotracheal Intubation & Mechanical Ventilation

6.   Mechanical Ventilation - using a portable ventilator until & while transfer is arranged and executed

7.   Non-Invasive Ventilation (NIV)

8.   Emergency Airway Intervention for Emergencies

9.   Nebulized Administration of Medications such as Bronchodilators

10.     Nasogastric or Orogastric Suction

11.     IV Fluids & Medications

12.     Defibrillation using conventional Defibrillators or Automated ones (AEDs)

13.     Cardiac Monitoring and Pulse Oximetry

14.     Chest Tube Placement for emergencies

15.     Lumbar puncture

16.     Initial management of epistaxis and nose bleeds

17.     Pain management and Moderate Sedation using Ketamine, Midazolam, Propofol, Fentanyl, Demerol and similar medications

18.     Administration of intravenous life-saving or emergency drugs such as thrombolytic therapy for acute cardiopulmonary emergencies, anti-arrhythmic for arrhythmias, vasoactive medications for hypertensive emergencies and congestive heart failure
